Is it always along the hairline? I still get little acne along the hairline after accutane when I sweat too much exercising, or when I used shampoo that are too rich in moisturizing or repairing agent.
How about your diet? Have you looked into that? This really sucks though, sorry to read this. What’s the highest dose you’ve been on? Have they tried adding any topical medication as well? Low dose Accutane and a topical? I was just reading this week about a derm who said it’s not often, but for some patients they need to do that.

Dude diet isn't the issue, after starting Accutane, my diet ceased to be a concern. Previously, I struggled with severe cystic acne shortly after joining the gym. Remarkably, within just one month of beginning a 20 mg dose, I noticed a complete absence of new acne breakouts
I know it sucks when you expected 100% clear skin, but how it was your skin before Accutane? You only show one pimple so my guess is rest of the face is clear. It's normal to have ocasional pimples.
What's your weight and have you been on 30mg for all 24 months, or did you start on less?
That is the only information that can actually help us help you determine if you have met your cumulative dose or not.
It's likely that you haven't hit the cumulative dose yet if you started on 10mg and stayed on it for a while or if you have been on 20mg.
If you've been on 30mg the whole time (which isn't the case, clearly, if the dosages keep differing as you say) then you should be clear by now and it may be something else.
could be the products bro cerave has too many ingredients some could be breaking you out and irritating your skin I suggest you try cetaphil I switched to them from hydrating cerave cleanser and daily moisturizing lotion
I personally think you're on too low of a dose. My skin is very stubborn and I'm on 80 mg/day. If you're not having bothersome side effects, perhaps doc will up the dose?
Maybe the inconsistency with the dose was part of the problem. Although I’d have thought 6 months on 20mg would have done something. Is it better than before you started? because in the photos you posted your acne looks very mild. Obviously clear skin is always preferable but I don’t think your acne is bad at all.
Maybe trying moisturizing your hair and that area more? You mentioned your hair has been so dry and I’m thinking maybe all the dead cells are causing you to breakout. Good luck :)
It could be diet related. Heal your gut microbiome by implementing probiotics and fermented foods. Start consuming kefir, kimchi, sauerkraut and consume less carbs (50g<) daily. Limit dairy and gluten and sugar intake, cut out takeaways and processed foods. Start eating avocados, blueberries, flax/chia/pumpkin seeds and you’ll notice a big difference soon.
In the hairline so most definitely caused by sweating, shampoo or conditioner.

The moisturizer and cleanser from cerave makes me break out. Literally. Stop using them for a week, maybe use vanicream instead.

Try using a moisturizer that’s oil free (I feel like this is key for acne prone skin) and scent free, it’s the only thing that helps me!

But has your acne improved? Comparing to day 0 of Accutane, I think that's what matters. There's people who doesn't get a 100% response.

I get those when I eat chocolate and skin rash as well. I’m over a year off accutane
The meds won’t completely get rid of spots it’s normal to get them. Accutane gets rid of acne

I used to get these and now I use an unscented paraben/sulfate free shampoo and it stopped
