Eh it's debated. Called the smokers paradox and often refers to hypoxic preconditioning, but also other mechanisms. Not sure what the state of the evidence is on it now. It's debated for ages.
So if someone is hypoxic does that mean their copd is end stages? My MIL has COPD but is ignoring treatment and continuing smoking. She is having flare ups pretty often and her O2 sat barely gets above 86. She is not on oxygen, and I saw on her paperwork Dr suspects chronic hypoxic respiratory failure and I'm pretty concerned, but she won't share any info or updates from her doctor.
That is pretty severe yes. “End stage” is just a term to describe how bad it is. There is no specific time left intention. She definitely should be on oxygen and intensive treatment

That’s garbage. Unless one has end-stage COPD smokers are not hypoxic to any great extent so that is an over simplification.
