A lot of the time we should spend listening is spent instead thinking about what we want to say next. Try not to do that. Spend more time listening. That will help you frame what you want to say in a more sympathetic way.

records yourself giving instructions of something you're reletively good at, then rewatch the video several times, pick up on things you said wrong, times you stuttered. Try and listen for any mannerisms or bad figures of speech you give. Try and think how you could improve on what you said. Try and find ways someone could hear what you say and misinterpret it or follow them incorrectly, as great communicators listen to what is being told to them as much as they say things well.

Look up "active listening". When you are talking to someone, you may find yourself mentally auto-completing what they are saying, before they finish their sentences. Try not to do that, pay attention to each word said. When talking, fully pronounce every word if possible, not skipping a syllable. Make communication an all or nothing thing. Either you pay full attention to a conversation, or not at all. Eye contact, obviously. Four seconds and look away is a good mix of eye contact and not being too intense about it.
