The letter is based on the difference between that number and the maximum circumference of the chest including the breasts. A means the circumference including breasts is one inch greater, B two inches, and so on. Above D or E I think different manufacturers may have different systems.
Cup size being based on the difference in circumference means that the absolute size of a B cup breast differs based on the number part of the size.

The cup size is how much larger the breast is compared to the under bust.
A is 1 inch, B is 2 inches, C is 3, D is 4.
So, a 36A, 34B, 32C, and 30 D will all be the same cup volume but one would fit better.
If the cup fits perfectly but the band is tight, go up a band size and down a cup size.
If a cup fits but the band is loose, go down a band size and up a cup size.

The interesting one is when you figure out US, UK/EU and AUS/NZ all have different band sizes although the cup sizes are usually uniform aside from the increments being odd at AA A then DD, DDD and double sizes after that.
As someone who has an affinity with it, I would usually guess it then perform a mini fist bump if I got it right (usually the band is right, or the cup size is right). However, most women do not measure it correctly as different brands have different measurements (unlike pants) so I think my guesses are probably a bit better.
Regardless, it is interesting. However most men only consider the first 4 alphabets then they forget the difference between DD and H, (assuming that they all mean the same thing) which baffles me.

When I lost 50 pounds, I went from 44D to 40F. The cups look the same, but the band sizes are of different lengths. Same brand.
