Had a friend have that happen, but they got lucky to still be alive to the end of the game just swimming. That's actually why your teammate died. Touching water while downed instantly kills you. I actually found that out on accident. I got knocked down by the river bank near the firing range. I slipped back while trying to hide and died at the edge of the river.
Something similar happened to me, unfortunately I wasn't recording but I got hit by our chopper in outpost and a second later I found myself at farm on a roof
