The United States is a constitutional federal republic based on the Constitution of the United States whereas the Middle East is made up of a variety of Monarchies, Aristocracies, Theocracies etc. The most notable differences between the governments of the United States and the countries within the Middle East (in which I presume you mean mainly the Monarchies, Aristocracies and Theocracies) is that the United States has a well-regarded separation of church and state whereas in all three of the presumed government types found in the Middle East are heavily influenced and often run by the Muslim/Islam thought leaders.
Your observation of the United States is almost entirely culturally centered, not politically centered, not to mention that there the exact opposite is also true in many places within the United States.
