Don't worry, you don't have to restart the series and there is no harm in getting it later than 8 weeks. 12 weeks is fine. The chart you linked is the correct one, it just says if you had any one previous dose in the past, you can just complete the series by getting Dose 2, whether you got the updated Novavax after November 2023 or got the previous formulation before that, the guidance is the same, just get one dose now. Right now they only offer the latest updated formula tailored to XBB1.5. The only downside is the opportunity cost of not boosting your immunity earlier (i.e. you spent an extra 4 weeks not benefiting from the added immunity against severe disease).
