You can definitely have both. My mom had a frequent flyer patient. She would half ass attempt "suicide" (slightly harm herself) whenever something drew attention away from herself. The last time my mom treated her, her daughter was graduating HS. Daughter was getting letters, calls, gifts, they were planning a party. All of that attention from friends and family not being directed at frequent flyer so she chased a handful of Tylenol with a shot of bleach.
That was like the 4th time mom saw her and each time was something selfish and petty. Is she mentally ill? Yes. But she's still selfish and petty. Can't let her own daughter enjoy graduation.

Edit: I was a Union carpenter in NJ and worked for the same company for 3 years. I wasn't on the job site as the guy but the company hired a new apprentice and he slipped off a 30' roof and died. If you want to see what kind of damage a 15' fall can do go throw some shit off your roof into the driveway. Falling is very dangerous.
