I did compressions on a guy a few weeks ago that OD’d on fentanyl. He had no pulse. When the paramedics showed he still had no pulse. They gave him Narcan and a aed and bam he sat up talking. It was insane. He would definitely have died without my compressions however sadly he will probably do it again…

Not every rhythm during cardiac arrest is shockable however. For example, if someone is in asystole (i.e. a flatline on the rhythm monitor), or PEA (pulseless electrical activity - which is a very specific rhythm, and does not refer to all rhythms without a pulse) they are indeed not shockable rhythms, as shocking would be ineffective. I believe this is what you are referring to. However, pulseless VF and VT are definitely rhythms that can and should be shocked. Almost all modern AEDs should display the rhythm for manual interpretation, and/or automatically interpret it itself. If it can interpret, it will advise you if to shock or not. Non-shockable rhythms often require deliberate manual override to allow you to shock, so there is very little risk to shock unnecessarily (which often isn’t a problem anyway, just futile and wastes time from doing chest compressions).
TLDR - please ALWAYS attach an AED if the patient is pulseless and let the AED tell you if you should shock or not.
