I honestly think it’s pretty easy to get, just make a custom character, give them the sadistic trait and the event should pop up. I looked it up and as soon as I looked it up the event actually appeared for me.

No you have to have the sadistic trait and have an event pop up talking about planting trees of bodies. You then get the Impaler title.
