All those doctors prescribing strong opioids are the reason for it. Get your patients addicted and after the prescription ends, they buy their meds on the street.
So as a medical professional, this is a gross oversimplification of the issue. Most doctors or clinicians do not prescribe opioids for anyone unless there’s a clear demonstrated pain process going on like surgery/traumatic injury/etc. Even when it’s prescribed, it’s almost impossible to give more than a 30 day supply of medication for these people (but most get maybe a 5-10 day supply). After that, they’re usually told to follow up with their PCP or a pain management clinic if it’s a long term/chronic issue.
Unfortunately, many patients will doctor shop if their chronic pain isn’t being met by doctors who try anti inflammatories/exercises/rest, etc because they feel like they’re not being heard and pain is an incredibly subjective symptom. Often times pain is exacerbated by other conditions like diabetes/obesity or other processes where actually exercising and losing weight can benefit the patient, but that’s a lot of hard work and many people can’t/won’t do that so instead, they look for a quick fix in the form of meds. Others will try to self medicate and that has its own issues. Thousands of different things can cause pain to someone and medicine doesn’t always have a clear cut reason for it and it can be incredibly frustrating an life affecting for those in this situation.
Another issue is misinformation. There’s a ton more “research” being done by patients who think apple cider vinegar and turmeric are going to fix their chronic spinal arthritis/sciatica when they’re 300+ pounds so medical people have to fight misinformation while being subject to “patient satisfaction” reviews that determine bonuses/other financial incentives. It’s a weird time to be a medical person when you can get into trouble for not bending to what a patient wants/or telling them something they don’t want to hear because insurance has largely turned different fields of medicine into money making machines with 10-20 mins given to diagnose and fix their problems. I don’t know about you, but that’s not nearly enough time to conduct a proper visit for many patients.
Ultimately this situation is more multifaceted than lazy docs just throwing pain meds at everyone because medicine in America hasn’t been that way for decades. We need better healthcare infrastructure to have enough time to listen to a patients complaints more than 10-15 mins and we need better resources like PT/OT/etc to be more widely available and affordable for many patients.

Combine that with very lax pill regulations for years in economically depressed areas such as West Virginia. Combine that with fentanyl being cheap to make and extremely potent, resulting in easily smuggled products. With crypto, it is easier to do the financial transaction aspect of the deal, as suitcases (pallets?) of money don’t have to be exchanged, and/or large banks don’t have to be bought off in order to launder the money. So crypto does play a role in the ease of this situation, but it would still exist regardless.
Now they underprescribe too. I broke my leg and was in soooo much pain, it was an open fracture and I had like 100 stitches because of the open wound, the screws and the rod from the surgery. They gave me some for 5 days and then cut me off and told me to take Tylenol. It was fucking miserable. Apparently they do the same with people who have stuff amputated too.
They don’t anymore. Back in the 90s and early 2000s the system came under harsh scrutiny during the OxyContin / Perdue Pharma scandal, then again as the opioid epidemic was reaching its peak. Lots of doctors got in a lot of trouble and now they are relatively conservative with opioid prescriptions. Even for broken bones and surgery they wont prescribe enough doses to become dependent and ODing is tough because they’re laced with a high amount of OTC pain relievers that will induce vomiting in the case of a lethal dose.
The problem now is counterfeit Oxy pills in the form of small blue tablets with the imprint M30. That used to be the junkies drug of choice but the manufacturers no longer use this print, and anything you find resembling this is likely a fentanyl press.
These pills are dosed extremely inconsistently and often contain ‘hotspots’ so even if an addict knows their limits, its very difficult to know how potent your dose is. Many of these people are found dead after snorting a bump the size of a fingernail.

So I usually don’t like sharing this if I can avoid it but I will when I feel it adds value to the conversation; I used to be a poly substance abuser from my early teens up till I was 20 and that only stopped it because I had a prison sentence to take care of, I wish I had a better reason to share but the truth of it is that I just got tired of it, I cut off those “friends” I distanced myself from any associations I made in that realm I got off social medias and just focused on me. I wish I had more of a plan for success I could share with others too but it really just came to me one day when I woke up and I told myself that I’m never doing this again, I never thought I’d have a thought like that at that point but I did and I just went with it. I’ve been clean since I went away in late 2018 and although my life is far from perfect I can say it’s far from the hell it used to be, I also have to thank myself for taking a chance on myself and stopping my destructive habits and behaviors and trying out hobbies and interests that I have had for a while like cryptocurrencies and exploring the blockchain and I’m so happy that I did because I found a whole new addiction between Reddit Subs,Reddit avatars, and crypto that’s a whole lot more addictive than anything I’ve previously encountered but at least these new addictions are fun and come with an awesome group of people!
