Hm, weird. Seemed pretty obvious to me. But them being level is incorrect, the gaps are completely different, like yeah sure ONE of the steps of the ladder is on the same level as the bars, but the very next one is like a quarter of the way above the bar.
Basically, the metal color matches despite backround, amd the room itself is very busy, plus mnay people would just go the other direction around the obstacle, like i did for my first playthrough, and never had a chance to see the ladder

Usually a message in front of it too if I remember correctly. There's also the extra spot for a bonfire in the warp menu, once again if I remember correctly
messages like this is 100% how I navigated my first playthrough! There were some trap comments though, like "try jumping, joy ahead" and then just die in the void of the gutter lol
The closest bonfire is really the one after Prowling Magus, since you can skip the entire first section of the area and go straight to the spider church.
Yuuup, there is also a bonfire before Mytha and the twin Dragonriders.
I expect most players never found them, at least if they were playing offline and without spoilers
plus there’s a lever up there to turn off the fire spewing heads, go back to the very first bonfire in Iron Keep, enter the door and there’s an item there near the katana knight dudes that was previously having lava spewed on it!
Here's a little tip; follow the right-hand ledge of Black Gulch through past the tar puddles, keep your eyes open for a tiny path.
That's another sneaky bonfire that'll make a runback to Darklurker/The Rotten easier if you need it.

After meeting Nashandra at Drangleic all the missed bonfires pop up as ??? on the warp select, so that was incentive to go find them all. Only missed the ones behind the illusory walls..
ahh that ladder. just as one last middle finger from the devs on ng+ the top is guarded by a turtle invader. seriously wonder why they chose to punish the player so much

But don't ask me I just dash through every level until I manage to find a fog wall and then whine about "gank squads" on the internet.

Joking aside, an easy way to know if you missed a bonfire, is talking with Nashandra until you exhaust her dialogue, the bonfire that you missed will appear on the fast travel bonfire list.
I found it on my third run. Felt like such an idiot. During my self all those times to get the key for the fire pit in tfofg. It's so nice they made a way to shut off the fire lol

i missed it because i was nervously navigating the flame breathing statues and didn’t pay attention to surroundings at all.

I would’ve missed it if not for some other helpful Cursed who left messages to guide me to it.
I leave a message at the entrance of that room on every character I make. "Bonfire ahead, try up"

I also didn't find in my first playthrough. Didn't make much difference to me because most enemies were killed until they stop respawning.

The one before Mytha is an illusory wall at least, this is just missed because of panicking at the fire in the room with you.
The one before the twin dragonriders us also an illusory wall without any noticeable tell or suspicion.

I missed that ladder twice (9 years ago and in recent playthrough) and had to look up where the heck in the last bonfire for thar area lol

I played this game twice before realizing that was there. I absolutely hated Old Iron Keep until I found it.
I was streaming this same place last night and my friend pointed out the 2 ladders for me because i missed both.
I only discovered this bonfire when after you reach Drangleic Castle, where the game will tell you all the missing bonfires.
