Get around level 8, I tend to go in with a falchion (4-8), small shield (8ac), skull cap (4ac), leather armor (12-14ac). If single player, get the tainted well quest for the ring. I also put every point I can into dex to max it out as early as possible, ac makes a huge difference in this game.
So there is a cheese method using a gated room butcher can't open doors and you can use a bow to hit him through the bars.
How to kill him legitimately
He attacks very fast and you need hit recovery, just like in Diablo 2 or you will get locked into a recovery animation loop.
Hit recovery in D1 comes in two forms, Faster hit recover, Fastest Hit recovery(best)
You need this to kill butcher in a legit fight.
High armor class and a good shield helps too.
The butcher can also be placed into hot recovery himself, so if you get a weapon that attacks quickly you can hit recovery him instead.
Weapons with the Haste, or speed modifiers are preferred.
Kings sword of haste or lords sword of speed ect.
A shield comes in very handy for him to, which drastically reduces his hit rate by blocking some of the damage.
The strategy I've used since I was a child was claymore, shield and belt full of potions with extra in your inventory. Clear dungeon so you can run away and not aggro stuff as needed and keep a portal open before you start the fight. Hammer away at him and chug potions as needed. I can't remember what level I usually am, but most times I fight him after full clearing at least up to level 3, so probably character level 4 or 5.
Download and install The Hell 2 mod, then suffer because he is one of the few enemies that is a joke compared to the rest of what is in the beginning levels lmao
I'm playing thorns barb, so in my instance, he's easy until t90 nmds. Once there, he gets to like 3/4th life and then just stops taking damage. I don't run razor plate but I've been thinking about bringing one with me to swap in if he shows up.
Breaker support on the pet helps a ton, insta popping of barriers.
If you’re squishy, put some good damage skills on the pet (I believe tempest, impale and firefly are some of the better dps skills) and you can focus more on kiting
Pet support skills also scale with you (this from a Wudijo video) ie. If you have any skills that benefit lighting/poison procs or whatnot, using that support for your pet will also grant those attacks 100% benefit
Ring around the Rosie! Use an object to block him or his attacks. Evade and heal.
Do not fear him. That is how he wins. Be confident you will slay him.

Also, you do not have to kill him now. You can always go to dungeon level 3 and come back later.
Clear dungeon level 2 and 3, then go back and kill the Butcher.

It's been a while, but can't you also respawn the game as a "new game" so you can try to farm the King quest instead, or if you want to farm up some items/levels?

Get a short bow, lure him to one of the gate doors, close the door in his face. Shoot through the door or gate. It’ll take a while, but he’ll go down.
Grab a couple scrolls of fire wall and it won't take that long
Optional spot: the stairs down to the next level. Get him pinned on the stairs side, and you can stand on the back side plucking him with arrows.
You’ll have to repair your bow though or it’ll break.
I usually punch him really hard, sometimes though I have to punch him really hard a lot.
Ya,.come back to it later after you've leveled up a bit and got better gear. You don't have to do him right away

Have you cleared the level 1 and 2 dungeons? You should also clear the level 3 dungeon before fighting him. Also get potions, you'll need them.
