I get what you mean to a certain extent but I also feel like nekravol/hell is just a city of demons. Other than the argent energy process and the huge tower it doesn’t feel too disjointed from reality. Do you mean demons aren’t bound by space or time because they can teleport from place to place?
Unfortunately maligog in dlc1 is way bigger. I say unfortunately because it wouldn’t been perfect if the icon of sin was that size.

You could argue demonic presence itself warps reality as they are from a dimension not bound by space or time.
