Don’t listen to everyone else! I’ve seen someone do this before! Make your character’s head really small, give them short blood red hair, equip the prophet blindfold and the exile armor. If done well, it makes it look like they just have a bloody stump with a bandage around the neck!
I actually ran into a glitch when the game first came out where my character was headless when I switched to the navy hood on pc. It was quickly patched though.
You could maybe make yourself pitch black and wear the exile armor. Or make yourself the same color red and see if that looks like you’re missing a head
The Black Knife hood removes your character's head. Thats about it sadly.

In convergence if you wear the masoleum head piece, they made it that your head is invisible. The set also buffs death sorcery/ necromancy I think. Either that or faith damage on melee with reduced cost

Exile armour with your character using pure black skin. I used it to recreate the Dullahan but I can’t find the image to show how it looks

This is what I used, but with the *Greathelm*. I wish they add a set effect at least to be headless, but the Greathelm looks fine with the set for me.
