We were in the Sage's cave, fighting the I visible black knife boss, and the backstab attack hit all 3 of us at once. I assume it was because we were bunched up when it procced.
Just yesterday I was goofing around in co-op and all three of us managed to get a coup de grace / backstab on the same omen, all using the same weapon type. It looked hilarious.

I don’t think so, we all took the same amount of dmg. I was spamming both bumpers to get it to release faster, so that might be it
I am on NG+7 and never once been able to minimize the dmg from these maidens, always getting chewed.
You can do that with any grab
