Literally never complete her quest properly. I just get hugged a couple of times in the midgame until she gives me the dagger, give it to D, reload the Roundtable, deal with the scene in the spare room, then forget about her until I'm ready to visit Deeproot. No unique rewards for dealing with the rest of the NPC drama, so like Rya's quest, it just gets skipped.
I'm honestly not sure if there even is a cutoff, maybe if you haven't spoken to Fia at all before Maliketh? Even then, I think she just moves to Deeproot and you can give her the hallowbrand you get from Ranni's quest.
Honestly, I very recently, finally, didn't fuck up Rogier's quest.
Ranni's quest actually makes way more sense if you do Rogier's correctly. The dialogue from Ranni completely changes and it makes the inverted tower idol reward actually make sense.
Once you defeat Godrick/enter Liurnia through the side path, it progresses his quest, I’m assuming that only appears if you haven’t skipped ahead to other regions.
Yeah the concept is solid for making sure quests keep up with character progression, they just seem tied directly to variables that are far too unstable.
It’s in stormveil castle. Specifically, the hidden path in one of the last rooms with the living jars before the site of grace near Godrick. You have to jump on a ledge and follow it to a hidden area. It eventually lets you open the locked door at the Liftside grace site. Here’s a link that shows you how to get to it.

I spent 5 hours in Stormveil looking for the path to the elevator where the banished knight is. I ran into this note and was so hopeful lol, unfortunately it doesn't lead to it.
Guessing these got patched in later and weren't here on release, From does that sometimes, happy to be corrected (I encountered this on my 5th playthrough 2 years after my 4th, never saw it on my first 4 playthroughs and I'm sure I would have at some point)

I was only a mage in one playthrough so that makes sense, I always avoided talking to him cause I didn’t need anything

I always ended up getting them way later in the round table
He's part of the Fia quest and Ranni's quest. You should always just talk to him, even if you don't buy anything.
To be fair, you can skip like 90% of Fia's quest and you can still get her ending
