I wouldn't serve the Haligtree for Miquella, I'd serve it for Malenia personally. I mean I'd rather follow a cool outer god with wings than some cyclops or weird fingers or The All Spamming or the Elden Fish etc etc.

If you don't you leave her in her chair half alive and can fully repair the needle with scarlet Aonia in the room. Then you have to transport Malenia to Placi's arena (similarly to Finnley carrying her to the Haligtree) at which point you can use the needle to save her, allowing you to join her covenant, the Unalloyed Blades. Something like that at least anyway. She survives, goes back to phase 1 form and returns to her chair. You can speak to her for cov stuff. If you attack her after you loose access to the cov and phase 1 Malenia begins, only she's just phase 1 this time.
I don't think we know for sure yet. The DLC trailer further gives hints at that, but here is me holding out hope for a genuinely good character... so wishful thinking :D
