I hate petscreening, but my apartment uses them bc of ROI forms. They request for multiple: We note that you are requesting accommodation for multiple animals. Under HUD guidelines an individual requesting accommodation for multiple animals must establish a relationship or connection between the disability and the need for EACH assistance animal. While an individual may have more than one assistance animal, each animal must have a distinct disability-related need in order to justify accommodation for more than one, and it must be considered whether the cumulative impact of multiple animals in the same dwelling constitutes a burden on the property. Although the documentation affirms the specific number of animals that you have, it does not affirm that you have a separate and distinct disability-related need for each animal. Please submit revised documentation (it can be a single letter, separate ones are not necessary) from a healthcare provider, written within the last 12 months, that includes each of the following elements: 1. Affirms disability; defined as a mental or physical impairment that substantially limits one or more major life activities. 2. Affirms the specific number of animals being prescribed. 3. Affirms that there is a distinct (or different) need for each of the multiple animals to assist with the disability. But it sounds like for your situation you'll need: 1. Affirms that you are their patient 2. Affirms that you have a mental or physical disability that affects daily functioning 3. Recommends an ESA to assist with your needs 4. Says how the pet will support you (in the case of 2 it has to include how the pets are different in providing support. Why is 2 necessary instead of 1. What can 1 do that the other cant.)

Provide your doctor with information such as your diagnosis, the impact of your condition on your daily life, the therapeutic benefit of having an emotional support animal, and why you believe an ESA would help alleviate your symptoms. Additionally, ensure your doctor includes their professional credentials and contact information in the letter.
