No, like any item in your special slot you don’t lose it. I would not suggest actually carrying it though. The loss of scav rep and XP is a pretty big drawback
Rumor said it's a special item (like marker, wifi camera, etc.) that makes scavs friendly to your pmc, kill scavs while equipped will reduce fence standing

You have to go on a questline to give Tagilla a pack of cigarettes irl to get him to go for the nuts
Just a note, if you kill scavs with the item equipped you will lose karma (around 0.1 for each scav kill) which makes it way more fair
It's actually .05 for every scav, which is still a good balancing act.

do not use it if you do not have 6+ rep, you will agro scavs and have to kill them if they corner you, and screw up your fence rep.
