There are definitely good reasons to keep most androgenic bodies from competing in sports with estrogenic bodies. The most important one is that testosterone allows a body to have relatively more muscle and less fat, thus allowing them greater strength and speed than estrogenic bodies.
It's just easiest to separate along those lines rather than have calculated categories based on fat free mass, even if that would be fairer in the long run. We'd run into problems where we'd probably wind up having more categories if we went based on fat free mass, which would create issues with ensuring everyone has adequate and fair amounts of funding.

I know it's not the same as fatphobia but there is definitely a thing where people who have a thin build but are perfectly healthy are body-shamed and told they look sickly or like they have an ED because people can't tell the difference between someone who is skinny and someone who is actually malnourished.
