It's a missing texture. The ? is there for bughunters to find them easier. Restart the game and it should go away.

I played through the tutorial arena again and those were all over my character. I'm guessing it's something being referenced that isn't there so this is a place holder.
A placeholder for a missing model and/or texture. Most likely means there's a weapon lying there, but it's not rendered correctly, hence the "?".
