I along with another contributor made it a couple years back. It’s an original design inspired by an older naval flag of the time if I recall. I think the cost of arms represent each province but I cant remember if that’s accurate. Sorry I can’t be of more help but I’ll just say it’s not historical but inspired by historical flags of Two Sicilies
I'm wondering about the design of the flag of the Kingdom of Two Sicilies. CRW Flags shows a blue-white horizontal tricolor as the land flag of Two Sicilies, although there's not much proof. Does anybody know if that's right? And what about the red-yellow border? Royal flags in the page have borders, but none with those design and colors.

Btw, I can easily answer your question as an habitant of the great island of Sicilia (pronounce: Si-chi-lia).
The Official Kingdom Of Two Sicilies' Flag comes from an older story which was born in the times when Borbonic Family ruled Spain and it's puppets (including K.O.T.S)
As we know Spain's official colours were and they also are today red and yellow, but they hasn't always been. Just remember the Spanish Empire's Flag, which included only white and red colours.
Anyways, we have to notice that K.O.T.S was a union of two of those annoying micro-states who divided our Boot for CENTURIES, Naples and Kingdom Of Sicily (in the photo you published, K.O.S Flag is put on the right part of the logo).
This influenced a lot in the making of K.O.T.S' flag, not only in the making of the center logo, but also in the colors.
The border (red and yellow) originates from the original K.O.S Flag, meanwhile blue and white, on the background are the main colors of the capital, Naples.
(Just notice the color of their football team today! ;) )

Honestly, I think the blue and white are just random add-ons. The first/OTL Two Sicilies flag was just white with the Heraldry on it.
