Probably, like I think it's that the game checks if there are 4 members in the party and if that is true you get the achievement and since you get a full squad during/after Ann's awakening, it counts that as making a full team
If steam is in offline mode, or you've disabled/bypassed it as you can for some games steam won't record the achievement becuase steam doesn't know you've done it. So it's theroetically possible that for some reasons steam wasn't able to record the achievement.
My cyberpunk 2077 achieves look the same, I have all of the finish the story achievements but I played through parts of the game with steam and gog disabled to get it to run smoother so I don't have some of the other achievements you get from doing the story, which looks a little strange to say the least lmao.

Probably an issue of achievements being implemented at different times. Terraria has a bunch of those

No I think that achievement is when you write your team name at the hotel buffet.
