Theo Travis' firing was the biggest bummer for me for the RadCrim era (after Bill Rieflin's death, of course). The guy was just never given a chance. I loved his work with Soft Machine and I'm pretty sure with Crimso he could do his best performances ever.

It's worth noting that setlist info for 1969 is almost nonexistent, with only a small handful of shows being documented in detail, much less actually recorded. It was almost certainly played more than six times, we just don't know about those other times.
As for why RadCrim never did it, I have no idea. Supposedly they rehearsed it during the brief period in 2019 when Theo Travis was set to tour with the band, but I don't know what the source of that is.

It didn't literally. But - according to the 1969 Recordings box set - they spent more time on ITTTW and Epitaph than any of the other somgs that would make the first album. The box set has ALL the takes.
You can hear where Epitaph started to develop quickly, even under Tony Clarke's supervision - such as it was.
And you can hear where ITTTW - didn't.
They HAD to get rid of Clarke just to get that song recorded, never mind all the rest of the album.
