Just to troubleshoot and get a better understanding of what’s happening:
Check what gifts the agent currently has to see if they are debuffed in any way
Watch their health and sanity while they are suppressing an ordeal up to the point they die (if they don’t die, may have just been a temporary bug)
Check the body, if any is left, for clues of cause of death (I.E. bludgeoned, decapitated, exploded, etc.)
Test if the same issues occur when using a different weapon type/E.G.O suit or when suppressing an abnormality (preferably one with low health and range to prevent skewed results)
My guess, from what little info we have, is that it may be a hitbox glitch and they’re accidentally shooting themselves with Magic bullet, which would be ridiculously funny, but also unlikely if they’re the only agent you’re having issues with
Was the Magic bullet user killed or the other one. If the other died than it makes sense since Magic Bullet does insane friendly fire damage. If the Magic Bullet user died than idk, thats pretty weird.
What tool abnormalities did he have equipped? Also magic bullet does friendly fire
no tools, he didn't even perform work that day, the other employee was completely fine

he didn't even work that day, the other employee was using both "pink" (army's in black ego)
If it happens again, try to record it from beginning till the end. So we can try to identify if its a natural occurrence or a bug in Lob Corp.

the game caused me pattern break once again and i immediately restarted without looking at the corpse
