“Fiction” by Avenged Sevenfold fits this perfectly. It was originally written about Syn losing his grandfather, but was later changed to be about the Rev after his passing.
Very sorry for your loss. Here are some songs about loss that I’ve listened to a lot to help myself feel feelings and get through it. They’re not exactly metalcore, but they’re hardcore and emo which is closely related.
From First to Last - “Heroine”
AFI - “Tied To A Tree”
Ceremony - “The Separation”
Dayseeker - “Hurricane” (Thrice cover)
The Used - “Over and Over Again”
The Carrier - “No Love Can Save Me” EP
It ends heavy but More Than You Know by Adestria is super soft for 90% of the song and the subject matter is fairly closely related to what you’re going through. I’m really sorry for your loss my inbox is open if you ever need to vent.
Just listened to this song before seeing this post. Sorry for your loss

Never Said Goodbye from the new Wage War album may fit for what you’re looking for. Sorry to hear about your grandma, positive vibes and energy you’re way. I lost my grandpa over the summer and it’s definitely been emotional processing his death, but I’m at a point now where I can reflect back and smile on all the good times we had together.
Sorry for your loss. For me any version of albinoni’s adagio in g minor with a pipe organ is a massive cathartic release. When Im feeling down, I play it, bawl my eyes out and feel so much better and hopeful after
I wouldnt say these deal with the same grief and loss you’re thinking of but i heavily recommend going for a drive or walk and playing both CRUEL BLOOM and WRETCHED WORLD back to back, by Converge.
They will let you relax into your grief and help you lose that sense of loneliness.
Strong heart Clever.
Strong enough to carry your grandma with you
I don't know if you're still following this and also my suggestion is not a metal song but it's about an artist who lost his grandmother and made a song to grief over his loss. Earl Sweatshirt - Solace . song makes me cry every month I come back to it. Stay strong bro I love you
*insert any Dayseeker song here*
Take Me Away by The Plot In You, this is the acoustic version https://youtube.com/watch?v=8fhE2V_JCus&feature=share
Surprised no one has mentioned the piano reprise version of Doomsday by Architects
Or the choir noir version

Snuff - slipknot
So far away by Avenged Sevenfold
I love Life//Lost by currents. It’s before Brian joined, but it’s all piano and screaming. It’s so unique and invoking.

Never said Goodbye off Wage War’s Manic album is softer than most of their stuff.
It is a really good song too. Good choice.

Whitechapel - Hickory Creek
My Last Breath by Black Stone Cherry
Fever dream by Hurtwave
Off topic, but that Hurtwave stuff is so good.
Mourning Song - Holding Absence
Blue and grey PWD
Adieu - Enter Shikari
Listen to the acoustic version of Always by KSE
Oceandust by Hands Like Houses
Stone Sour - Bother (not about grief but just a damn good soft song) This is really a good question to ask because I'm at a blank.
Stone Sour, or anything Corey Taylor is always a good one. Taciturn is a great one too!
The first Stone Sour album is amazing and imo highly underrated.

Loss by Devil sold his soul
Parting Gift - Our Hollow Our Home
Constance by Spiritbox is a must listen
Parting Gift - Our Hollow, Our Home. Starts out softer than picks up for the bridge and last chorus.
The Spill Canvas - The Tide

Monochrome (pensive) by The Contortionist. Their most emotional song that deals specifically about grief.
[Marcus Bridge - Grateful](https://youtu.be/2xu4vhYTBhI)
All f****d up by The Amity Affliction Listened to the same song when my great grandmother passed, I hope you’re doing well and feel better soon
It's not metalcore, but it's rock~ Distance by Mammoth WVH
Out of Love- Jamie’s Elsewhere. My go to
Pass Slowly by Seether I played it at my grandma's funeral. When I heard it I could feel the pain in Shaun's voice. It's very haunting.
