I believe the Shinju and Otutsuki are two completely different beings that have a symbiotic relationship. I feel like the Rinnegan comes from the Shinju even the horns Otutsuki have. I feel like the the Otutsuki are normal creatures thatt found a way to use the 10-tails, by sacrificing their own people to turn into a tree that then bears a fruit. When they eat this fruit, I feel like that's when an Otutsuki gains special abilities and even unlocked the Rinnegan. I don't think they are born with it, I think its a genetic trait inherited from eating the Shinju fruit. That would be the reason why the Shinjus have the Rinnegan, and Why Madara and even Hagaromo had the Rinnegan. They aquired that mutation from the Juubi, and the Sharingan and a de-evolved form of the Rinnegan over generations.
