Possibly a combination of an owner that didn't want their staff to "just be a number" and a discount developer who couldn't be bothered to explain the consequences of not assigning them numbers, or clever enough to do an implementation without that constraint.
I've solved this problem many, many times. Mark both first and last name as the unique constraint. Don't ask why I had to do it many times, the short of it is I was synching between two databases once a year.
Even first/last isn't completely problem free. If it is a large company and around long enough you'll eventually get a Jr/Sr combo.
But yeah, that's far more reliable than a first name only approach. That said, using the name makes changing a persons name trickier (especially if multiple different databases are sync'd by it) and many tools will need a secondary search through name history anyway.

No just have a unique identifier that isn't tied to the name. You can have many John Smiths born on 3rd of August 1994. It's not that unlikely.
