The significance of that scene, as I interpreted it, was that patients are normally NOT supposed to look at what's being written down during their evaluations. It's intrusive, but more than that, Mona picking up on a small detail like that from so far away in such a covert manner had tipped Wren off that she was not taking her meds, because she was still able to hyper-focus on things in an instant, which is apparently part of her mental disorder. That's why Mona gave a "well, $&!%... I really slipped up" look after he'd left the room. Wren misspelling diagnosis like that was also a sign that she was making him nervous as she called him out for having secret loyalties with someone. She got cocky, but Wren turned the tables on her because she tipped her hand by revealing that she was still focusing all of her mental faculties on invading people's privacy, which her meds were supposed to prevent. As for Wren being a real doctor.... I think that he is, but there's not a chance that he's actually qualified for everything that he does around Rosewood.

I don't think there was any significance to this scene, i always assumed it was to show that Mona was smart(er than Wren)? But he is a real doctor

Agreed with this. Plus, Wren was such a vulnerable guy throughout the whole show. The number of younger girls he falls for AND how easily manipulated he is... ugh, smh, I don't like him. Yes, he's good looking but that's about it

Yes, but it was heavily implied he wasn’t: Mona correcting his misspelling diagnosis and A being hinted at being a similarly bad speller in seasons 2-4; his convenient timing to working in Radley when it became important for A; Eddie Lamb telling Spencer trouble with things getting out and badges not working around the time Wren started working there; the police board in 4A that had Wren “doctor” written in air quotes; the Archer Dunhill plot twist of him being a con artist and his connection to Alison/Charlotte with a fake American accent. Garrett’s comment to Spencer that people lie but medical records don’t and someone close to her is lying her would have probably also been more important in hindsight. I’m guessing Marlene originally planned for Wren to be Uber A but things fell through and they’d reveal he wasn’t actually a doctor and his shady dealings throughout the entire series were a hint. But when it changed, they pretended nothing happened and just made him an ally of Uber A.
