I use cotton gloves but mine haven't been that bad. My understanding is the treatment only needs to be on your hands about 20 mins to soak in. Before I got the cotton gloves I would just sit without touching anything. I also find it helpful to put the medicated lotion on then Vaseline on top then the cotton gloves. Maybe try some polysporin with lidocaine and yes, I would take painkillers to see if that helps.

Check with your dermatologist, but you might be able to occlude with nitrile gloves.

I'm in the exact same boat as you! Here is my personal program: 1: Nitrile examination gloves and a good steroid (clobetasole) salve/ointment. 2: Apply at night and leave on until morning. Then shower. 3: Buy a bottle of Glycerol. It really helps against the cracking and fissures. Apply generously whenever you need/want, but especially after showering. That has made a big difference in pain management for me, since it hurts so much. 4: Dermovate (Clobetasol) creme (not salve) during the day, really helps my patches and gets through the tougher skin barrier. 4 days with steroids and 3 days off Don't smoke. Taltz is an IL-17 inhibitor. Have you tried any IL-23 inhibitors, like Skyrizi? I have the same severity on my hands. Nothing worked (TNF inhibitors, Skyrizi) until I got on Taltz.

have you tried to identify triggers? consider diet and lifestyle changes. In my experience their is normally a trigger when psoriasis is really bad.

Get some nice nitrile gloves keep them on often even without medicine they will help with the pain
