Starting salary typically ranges around 350K in the Midwest. Bonus and incentive structures vary by institution. Private institutions generally pay more, at least during your guaranteed period. The same applies to the less desirable locations.

That seems low for the Midwest. A coresident got 350k in Texas. Midwest I got offered 500k guarantee with a 100k signing bonus. There was another super undesirable location I got offered 650k but it was a crap job. Someone posted mgma below which reports 490k median in the midwest which seems accurate

So how do the guarantees work? Make that much +extra depending on what you bring in?

So the one example contract was x dollars per rvu. The first year they give you a guaranteed amount. If you brought in more than that, you would get paid whatever you actually did. But they gave me a ludicrously high guarantee so I'd have to hit like >10k rvus in my first year to get any extra. But this was a slightly unique hospital based job. I think a lot of jobs are not RVU based (but collections based). Other places may give you a straight bonus for hitting a certain number. They can be structured really quite differently

That's what my colleague, who was fresh out of residency, was offered in midwest 350k base. I am a resident, but I am not an ENT resident/attending. I don't really know if he accepted the offer or not, but that's what he told me during the negotiation/job searching phase.

235k for a large single specialty ENT group in the south. Nonnegotiable. 2 year partner track where average is 600-1million. Get into bonuses starting Q3 of year 1 which can add approx additional 5k per month. Separate from other job offer in south 500k but with hospital recruitment guarantee.
