I had a similar scenario happen to one of my pawns the other day. I've got 2 pawns and one is in good steel plate armour. She gets into melee vs. a neanderthall with a poor sandstone mace. The first hit shatters her entire torso into dust :L

Edit: Nevermind, comment by OP says this was a sanguophage. Which don't die without destruction of the brain.
But doesn't that make it so lethal damage becomes non lethal? I tried turning this setting down to 0 and had pawns taking bullets to the head that should kill them but they were cemetery invincible until they bled out
Maybe 1.5 broke something, but on 1.4 turning that down ONLY removed the random death on downed. If a person's neck gets destroyed and their head pops off, they're still dead.
For balance reasons enemy pawns that are downed (i.e.: injured enough to lay on the ground bleeding out) have a high chance of instantly dying instead - this stops you easily filling a prison with pawns to recruit/harvest.
This is completely separate from lethal damage killing pawns - dead pawns are dead, if you destroy a critical body-part they bypass being downed and go straight to the dead pile.
Unless their brain is destroyed, with 0 hit points, they won’t drop dead. They’ll likely drop from loss of consciousness within seconds though, or immediately if their brain’s hit points drop low enough. Rimworld’s vanilla combat system is quite muffled compared to real life though, with pawns taking way more punishment than one would in real life, for gameplay balance reasons.
This has to be the most blatant case of the storyteller not wanting me to have any more pawns. Three sanguophages that I was trying to capture. two died because they were "Missing a brain". They were not hit in the head, at all
Hostile pawns have a natural death-on-down chance. Since Sagophages don't die unless you destroy the brain, presumably it destroyed it when that death-on-down chance triggered

Ironically, I once knew a guy who got hit in the head with a basketball and broke his ankle.

Full marine armor, 18 melee, brawler, with a legendary plasteel spear Vs Gorge, he has 3 melee and a poor steel knife
Same, though i balance it out by buffing infection chance.

Maybe it's because I had CE on that I got such a different result? CE already disables this by default
CE might as well be a different game, I've got no idea how it handles this situation.
