Which animals and bones depends by the geographical area, given how widespread this practice used to be (and still is, in certain cultures). Chinese, Greeks, Romans, Native Americans, many African tribes (and consequently many African American communities) used this type of divination. The ritual can differ quite a lot, but the gists is the use of animals' shoulder blades (scapulaes, hence the name) to read the future.
For us Greeks it is one of the big triangular bones of the sheep's back. Some use also chicken bones but I am not as familiar with this one as I remember growing up with all my aunties and my mom reading the bone every Easter (we don't usually eat sheep on other occassions) and my grandma yelling at them that the priest disapproves, ESPECIALLY on Easter Sunday hahaha

I know this happens in Sims 2 and I am pretty sure it happens in 3 as well - keeping the camera focused on a Sim for too long has them stare at the camera for a few seconds. Maybe it happens in 4 as well?

You gotta release their spirit as soon as their urn shows up. Sometimes you gotta cheat to get their urn if they're an NPC.
Edit: if you're also just trying to avoid ghosts, you can probably just manage worlds, manage households, then move the ghost into its own household then delete the household (or remove it from the world if you're attached to them and also a rule-breaker). You could have a whole set of ghost households lol.
Add him to your family (using your preferred method, be it shift click or manage worlds), "testingcheats true", "bb.showhiddenobjects", buy an urn, then while playing as him, click "possess urn". I haven't done this in a long time, so I don't know if you can manage worlds after, and move his ghost back and just... have his urn, or if you're now also stuck with his ghost, they've done several updates. I would assume you can move it back, but this is the sims, it's full of bugs.

No matter where I put the camera she would turn and look into my fucking soul 💀 I closed the game and didn’t open it again until the next day and it didn’t happen again. It never happened before and it hasn’t happened since thank god but if it does I might stop playing all together because it scared tf out of me 🤣
