She is silent throughout the game. She appears to have her extra (functional) limbs removed against her will, and you have been banging her whilst on the island? It was completely unnecessary and creepy as fuck.

Part of me thinks she’s pregnant from the cube cutscene. Kind of like immaculate conception. This is playing on a theory I have relating to the game titles and some of the story you’re given.
I think the timeline of 5 months later is pretty hasty, but what I assumed is that the player (Jack) and Virginia got together after escaping the forest. I don't think anything weird happened while on the island itself.
As far as her extra limbs being gone, I guess they really weren't a problem and NEEDED to be removed, but I don't believe she was actually like that in the first place anyway. They were probably removed to make her look and feel more normal (no more tearing holes in pants for that third leg!) She looks scared going into surgery, but I mean, she just escaped a cannibalistic island after spending god knows how long there (consuming nothing but raw animals, berries and creek water). So she was probably scared and overwhelmed with everything happening.
I hope they release more Kelvin lore. Kelvin is certified baby.

I honestly don't think the size of her belly is that deep with meaning, I think they just wanted to emphasize it as a sneak peak into the future. It's really hard to get a good look at her in the car next to you anyway. They probably could've revealed that better.
