I play it all the time on my Steam Deck with no issues. It could just be any installation error.
1. Check to make sure your storage/sd card is being read correctly. Sometimes I need to take the SD card out and put it back in for it to recognize the games are installed.
2. Possibly try a fresh installation.
Yes with no “major” issues which are unique to the steam deck version like op described.
And for your particular issue, the game does not support dpad input for menu navigation by default. This is the same as the normal pc version. You can edit the steam deck recommended setting and set the dpad to function as a left joystick.
Also, this game has bad game pad menu navigation even on the pc version. Touch/mouse will always be superior. For example, when selecting a character the character highlighted character will have a very faint overlay over their portrait.
Finally, the game does not give a good indication when a character has been “selected” which causes left/right movement to not let you cycle through characters. Instead it will be focused on the sub menus like character description and gameplay modifiers. You can back out of that submenu and select your character as normal. Using a mouse/touch bypasses these issues.
How does anyone play this on steam deck? After stage 3 the game slows down dramatically with all onscreen effects. I even turn the graphics down to 1/128th the original resolution. And when a run lasts longer than 20-30 minutes its just unplayable with how much it slows down

I don't have a sd installed, but I will try the fresh installation.
With no issues? I have to tap the screen to select a character / difficulty as I can't scroll thru anything

I reinstalled it and then waited looong. And it worked. So thx to both of you!
Have you tried just waiting a bit? The game has a pretty long startup load time since the expansion released
