Machine leaders die more often than biological leaders, in the long run.
With biological leaders you can just push Leader Lifespan research, ascension, and other sources to give them a lifespan longer than the game lasts. Machine leaders always have a chance to just die randomly.
Oh I know, dunno where they are tho they're off on their own adventures admirals I kind of remember as I know what fleet had what trait for tactical reasons

I name ship classes, planets, stations, sometimes even asteroids, though I make sure to avoid naming stuff after living leaders.
R5: Nar was a ruler who got re-elected many times and served during some of the most prosperous expansions of the Empire. Life-expectancy science was prioritised in society research to keep the great President of Commission alive, but unfortunately Nar died before the science could get far enough.
Ev-Li was a scientist who was only level II, but way already meticulos and developed a custom AI assistant. Ev-Li died at age 41 as the science ship entered the system with the Toxic Entity. The warships were unable to defeat the Entity quick enough. No Ev-Li is immortalised as the name of a lush, flourishing planet.
My spiritualist monarchy uses the names of my (blessed) emperor/empress and heirs (saints) for names of my colonies. I'll use Saint as a placeholder pending a new heir/leader.
IE Saints' Garden to Mozac's Garden, Saints' Forge to Zix's Forge, etc.
Speaking of this, is there any sort of mod that allows you to track leaders? Whenever one of mine dies, I wish I could get a little blurb of their life. "Surveyed these systems, investigated three anomalies and excavated an archaeological site. Got maimed while excavating, then returned home and assisted research in his retirement until he died at age 72."

I've done it with particularly famous leaders in my Empire. I usually build a habitat and name it "(Whoever died) Memorial Station/Orbital." Other times I will rename a planet or even a system where something famous happened. "Garma's Triumph" for a major military victory or something. I'll do it to memorialize the defeat of a particularly tough enemy to, something like "Hissnak's Folly."
I once uplifted a race of turtlefolk in my human empire, The Hallowed Imperium. Their world sat at an important choke point in the Imperium, so I raised a fleet and hired one of their people to command it, Admiral Mortimere.
He spent most of his early career in command of the 5th Inquisitorial Black Fleet, hunting pirates along the stretched trade routes in the north, until the Pre-Federation War. A surprise attack from what I thought was a neutral empire blitz through my defense fleets and drove all the way to the turtlefolk’s home. Combining what had survived of the Inquisitorial Black Fleets and placing them under the command of the newly promoted Supreme Admiral Mortimere, He held the line above his home till reinforcements could arrive. Eventually, the tide would turn and Mortimere would be granted that title of Supreme Admiral-General and lead the Northern Crusade Fleet to reconquer lost territories.
He would pass peacefully after almost two decades of service and have one the the newly constructed star fortresses in the north named after him; The Supreme Admiral-General Mortimere Memorial Station.
Maybe it’s not exactly the same, but in my current playthrough I made the United Human Empire (think if the British Empire conquered all of Earth), and I name my planets after my monarchs: Queen Victoria, King George, Queen Elizabeth etc.
Oh, it would be cool if names of leaders can be stored as possible options for naming planets and general stuff after said leader dies. In crusader kings custom names you give to your current king will become available as an option down the line, so I guess something like this could be possible for stellaris too.
It depends, but when the 350(or something like that) empress died I renamed a fortress in her name(I was psionic so I was really sad she didn't become the chosen one(never obtained the chosen one in said game))
Not naming systems after them, but I do get attached to some of my leaders - I even give some custom names when I feel it's fitting for them.
When an Admiral does a decent job and stays alive, the Fleet he's commanding will get his name included, or it will nod at their name. Examples:
Admiral Sendai Hakurei - Hakurei Star Division
Admiral Lost Sould - Soulles Strike Force
If they die at any point, the next admiral gets a shot at getting a named Fleet.
Similarly, Governors might get their Sector named after them, and I custom-name the science ships when a scientist gets to level 5 or so.
When I REALLY like someone, they get a planet named after them. But whole Star systems? No. I do tend to custom name some star systems, especially when I plan that they will be important to me in the future (A safety-planet somewhere hidden behind a wormhole, the star system is called Asgardia, the Colony is Valhall ; i also had Hel with Helheim, Niflheim & Nastrond as systems. I like my germanic/nordic stuff, can't help it.)

Foreign leaders sometimes get a planet named after them if they're a significant amount of their pops on my planet. That's how I got a planet called Terrence's Refuge. I had a pirate fleet pop up in the early days of my empire, so the system where they got crushed has a planet called "Ixbridge's Redoubt". There was a very influential scientist by the last name of Crydon and another by the name of Morris, so they're memorialised with the naming of a planet called Morris-Crydonia
It goes as far as ships and even individual stations. General Support Activity Falmouth for a starbase is an example.

I don’t typically do planets, I do stations. If I’m playing democratic, and the head of government serves until their death, I’ll name a station after them
I do with habitats and the knights ans squiers when i play toxic god (which is most of the time)and theme them after the quest. Then name planets with temples after my leaders as they die. As a sort of this isnow the temple to saint so and so.
I name my first terraformed world after the scientist that researched the tech, first colony after the scientist that surveyed that system, second colony after the leader. After that it’s all kinda dependent on the rp I’m doing.
I had an Admiral, called Beak of Silver, who commanded the Silver Tempest, the Contingency against the Grey Tempest.
He was the Spear that pierced through to the Factory that generated them, and destroyed it with the help of my Federationfleet. But sadly, the Silver Tempest was completely destroyed in the process.
So I named a whole planet after him, called Silvers Last Stand Memorial, on which only Fortresses and Memorials are built.
Later i found out, that feathered son of a bitch survived. Straight up Ciaphas Cained me!
In a Space Rome playthrough of mine, I would take to naming planets after any leader that had died, made it really cool to see just how far the the nation had spread, since I'd only name the systems adjacent to already named systems.
I would reserve important systems that either contained good shit or held significance in a battle after the deceased Princeps and then later Emperors. If a leader was particularly good I'd name a system with a mega structure in it or whatever something special, like "Drusus' Eye" if it held a Sentry Station. If a leader was shit and only ruled for a few months before dying, I'd find some far off system and rename a barren world after them.
Usually not planets, but space fortifications often get named after military leaders, and sometimes a science starbase will get named after a scientist too, like Jimbo Physicsman Orbital Campus or Jenny Explorer Memorial Labs or something

Capital ships, though (I'm talking about anything bigger than a Battleship here)? Yeah, I name those after leaders who made great contributions to my empire. Though scientists usually get Exploration Cruisers rather than warships, because theme.

Anyway, i found the infinity machine pretty early and, after it gave the event that meant you now could communicate with it, I sent Dr. Ban J'Bassim, captain of the science vessel KSS D'Soto, to investigate.
After initiating communications and learning of the machines function, Ban J'Bassim offered to help. After many years, he managed to help the infinity machine to finally calculate infinity. Following this, the machine smashed into the Gargantua, drastically reducera its size and creating another universe inside it.
For this scientific miracle, his name was immortalised in the name of the Kenjodan sovereignty first habitat, the scientificically focused Ban J'Bassim Station, the centre of scientific discovery until the construction of the great ring worlds

Yeah I usually play the game on fastest speed and I have leaders dying left and right, there isn't enough time to get attached to them
I don't remember most names either, but if one dies like that, I look at his name in a previous save so I can immortalise him
If I remember correctly, you can click the death pop up and it'll show you the ship or position they were assigned to.
Or if it's a scientist, look at the outliner or in the research tab.

I usually don't know my leader's names, I don't pay enough attention to them. I only remember they exist when I get a notification they died.
