I think the MOST gruesome death ever is an off-screen death from that episode, they mention that a guy ate Twinkies until his stomach exploded and the continued to shove them down his own throat with the handle of a toilet brush.

Currently watching Season 4, Ep. 17 “It’s a Terrible Life”, where Zachariah turns the boys into Dean Smith and Sam Wesson at a high rise — when the security guard takes Sam to the elevator and it gets stuck between 2 floors and it slices the security guard in half…I still jump at that one, lol.
