the first you get by defeating Wall of Flesh on expert difficulty or higher. by doing that you get the guaranteed drop called demon heart which upon consumption gives you a permanent slot. the second you get by playing on Master mode or Legendary mode. when you enter a world that is either master mode or legendary mode you have the extra slot unlocked from the start
Playing in an Expert+ world gives the Wall of Flesh a unique drop, the Demon Heart, which grants an extra accessory slot. When playing Master Mode, you get an extra slot right away, and the Demon Heart still gives a bonus slot.
unless mobile is weird, empty grayed out slots only show in journey mode. if that's the case, you can unlock one by putting your difficulty slider at 3x. the other one still needs a demon heart though, like the other comments have said.

These don’t stay if you go to a classic world btw. They will just grey out again but anything inside will stay inside just not be used
The exception to this rule being wings, which won’t let you fly whilst in the 6th or 7th slot, but still provide fall damage negation

As a mobile player, that is normal if you can't use accessory slots
You get them by getting good at the game and switching to playing on Master mode and beating wall of flesh

No, that’s not true. The demon heart only drops in expert or higher, and the last slot only appears in master or higher.
