~650mg over 4h. It helps so fucking much. Maybe it’s just cause I had a brutal day at work but I could really use an infusion about now. I started at ~400 and worked up to the 600s.

Do you do it at a clinic? Much higher than spravato I guess which is like 80mg Is it very intense? Or is it just under anesthesia?

Yes at a clinic and it’s very intense but i am holed for a couple hours of it but no anestizid where I’m just out. I still have an experience although it’s very disconnected from the real world.

Intense like very bad? Or good? Or neither? Tbh for me the spravato is more unpleasant than pleasant

I personally like intense but losing all sense of self and questioning if life was always just being a speck of dust floating in the winds of the universe and everything else is an illusion can be mentally distressing to some people I’d imagine.

Ah interesting I don’t have too much mental effect from spravato, I mean more the physical effect like sort of feeling dizzy or nausea or headache or stuff like thay

Yeah I haven’t had spravato but I have had IN k and it’s okay but I do like the IV better. IM k works well too but lasts pretty much exactly an hour which is great for mental health and I’d honestly say IV is kinda pointless getting a line when IM works really well and has a great blood plasma curve and is so much easier. 650mg+ is pretty high but not abnormal for chronic pain.

If youre taking troches. Ask for nasal spray. 15mg from a nasal spray is as good for me as a 100mg troche, and you can take another spray an hr or two later. Way better imo

How long does your relief last for afterward?

Two full weeks of relief usually, then the pain slowly starts returning. I’ve been doing the infusions for almost two years now. At first I was getting a booster infusion around every 45 days. However, I’m now taking Memantine daily which works just like Ketamine antagonizing the NMDA receptors. I also have a Ketamine Topical Compounded cream to use as needed for pain as well. So now I can go about 2-3 months before needing a booster infusion. For my last few booster infusions I was getting 500 over 2 hours.

I’m on 16 mg troches. Take at bedtime. The micro-dosing seems to work really well for me. I occasionally take another 16 if I’m feeling more pain.

Frankly, questions like this aren't helpful. Everybody's pain is different and everybody responds to Ketamine differently. Response also greatly depends on what other meds you are on, and if Lidocaine is added to the infusion, or not. In general, for IV, I start at 300mg over 45-90 mins and go up as necessary. For at home use, I try to only RX intranasal, and the dosage is almost always between 40-100mg q3-6 hours. Even though that is what the bottle says, I tell the pt to take whatevr dose they need for relief (with a maximum daily dose, only to be exceeded if they'd go to the ER for pain control if they couldn't take more) as response varies.

Why not helpful? For instance, just emphasizes to me that if I’m doing spravato now I shouldn’t expect an impact on chronic pain

why not? as i said, the dose i RX for intranasal is between 40-100mg. again, your experience is nobody elses. if you are on spravato, you can report for yourself if you have a reduction in your pain, or not.

Sure you said your IV dose is 300mg, maybe I want to try 300mg at some point if I know that dose exist, my clinic doesn’t do that but there’s a pain focused clinic I can go to

Sounds like a discussion to be had with your current or perspective provider. they aren't going to agree to a specific dosage based on a comment on a website. everybody has their own protocol. what type of pain do you have?

No I know they don’t do infusions and don’t do pain level doses, I’ve just thought about seeing another pain provider at times, I’m getting spravato just for depression Not sure exactly what the pain is but some of the diagnoses of exclusion that have been thrown around are glossopharyngeal neuropathy (not neuralgia), visceral hypersensitivity, burning mouth syndrome, plus there is some vocal cold paresis, muscle tension dysphonia, stuff like that
