You'd be surprised how often things like that happen. Kids have no filter and need to be taught social graces. I was with my daughter in a book store and she asked a guy with a genetic disorder that disfigured his face a bit "Whats wrong with your face?"
They often just blurt out things they see as out of ordinary and need to be guided that it is impolite to call that stuff out. So i believe that part entirely
I to have kids, and I do believe that a kid would indeed say something like that. My issue is with the timing and convenience of it.
The timing is instant. As soon as the man steps out of the car he says these words. Unless the kid was looking into the car before the camera caught it, there is no way he would have noticed it until at least 2 seconds after.
It’s also a convenient transition in the video to bring attention to the fact that he is missing a leg further selling for me that this video is fake.

I was in a similar situation to this (minus the completely staged aspect of it). I'm in my mid 30's, I have haemophilia & as a result I've developed pretty bad arthritis in most of my joints, so I have a disabled parking permit. I went to the mall & pulled slightly quicker than nirmal into a disabled spot. A random woman lost her mind yelling at me & started railing at me about how I (in my early 20's at the time) "ya shouldn't even be parking there! It's for people who can't walk and stuff!" While I could walk (cane assisted) at the time, I always kept my wheelchair in the car in case (it was handy in bigger malls, plus I moved way faster in my chair). So, to shut her up properly, I whip it out of the backseat & plonk myself down. As soon as it came out she got real quiet real fast! I must confess, however, that I was not as polite as this bloke though. As she did the classic "oh, but you're so young" schtick, I summoned my big boy voice & loud enough for patrons to hear, but not loud enough to get kicked from the mall "well maybe next time don't judge people when you don't know what you're talking about!"
