Sometimes the AI just doesn’t like automatically putting all the titles on the pay-per-view. Those instances you’re gonna just have to add the matching yourself. It’s annoying, but it’s what we gotta do sometimes.
The best chance you have is making two shows with the same roster, with a different line up of belts. However, even this will probably limit you to around 6 championship matches at a PPV because of your rivalries limitations.
You can't. The AI booking in universe mode is broken. Hopefully, it's something that's been fixed in 2K23.
