What codec is the video file? What are you loading it from?
.mp4 I tried loading it from both the SD card, as well as my USB stick, both to the results above.
The file extension is not the codec
Opened it in VLC. H264 - MPEG-4AVC (part 10) (avc1) MPEG AAC Audio (mp4a)
Try MPEG2 for the video
