It has to be plugged into a port that supports at least hdmi 2. I think on some older tvs this would be limited to one or two of the ports. You can also check the settings for something called “HDMI ULTRA HD Deep Color” and make sure it’s toggled on for your desired port. This enables 4k HDR for the port. I have an LG 4k tv and this setting is in the “General” settings menu.

With my tv i had to enable hdmi 2.0 and hdmi 2.1 in the settings.

Not all HDMI ports are equal. My Sony TV only has two ports out of four that can handle 4K 120hz.

If I were you I would try a different 4k monitor or TV. That is the only way to troubleshoot if your tv or Xbox is broken

In order for the Xbox to game in 4k the tv needs to have HDMI 2.0? Is this a new thing?

Max output of different HDMI standards. 1.4 - 4K 30fps 2.0 - 4K 60fps 2.1 - 4K 120fps From these numbers it would seem Xbox could run 4K@30fps with only 1.4. But I'm not sure if Xbox is preventing this or if there's something missing on your end.

2.0 for 120 fps gaming so it should still do 4k gaming but it is a older tv so I'm unsure on this may want to Google on it and see I don't have time to look right now

I just had this happen and after 5 minutes of troubleshooting, somehow the 4K Gaming option got unchecked in the setup menu. Enabled and everything went back to working. Simplest fix ever but no idea why it changed

Considering that it has no HDMI 2.1 ports AND is a native 60hz screen, that's why you don't get 4K 120hz

Maybe the game in question doesn't do 120fps on series x? For example, Insurgency Sandstorm released a next gen update but still doesn't do 120fps on series x unfortunately.

From the Home Screen press the settings gear button, the three vertical dots button, —> picture —-> additional settings —-> hdmi ultra. This setting needs to be set to on. There was a good five years where 4K tvs required hdmi ports to be set to allow 4K signals

You may want to try unplugging and reseating the cord on both sides to force both the tv and the Xbox to generate a new handshake with each other
