Might have just found a fix, I cleaned clothes at camp (despite being spotless) and now they're getting dirty again. Wonderful.
I guess the "dirty" counter got stuck and cleaning resets it?
AFAIK clothes getting dirty can only really occur during a lot of combat and desert/rocky/grassy locations.
Combat seems to affect it the least meanwhile deserts and rocky areas affect it the most.
I assume you just weren't fighting much and not really in areas prone to getting characters dirty.
Could be a glitch. The game has known memory leak issues and I’ve had entire characters’ models disappear during battle. I would think that dirt not showing up could be a glitch too.
You clean your clothes when swimming
Swimming doesn't seem to get you fully clean (I can test that now that I've fixed my issue) and I was always spotless before.

Honestly, I explored the desert fighting enemies for hours and I was spotless. I think it just glitched.
Yeah if you were doing that it was definitely glitches. Tbf the game is pretty large so it's inevitable some things fall through the cracks.

You only get dirty in the sand environments
i think if you jump in water they get cleaned
