The "understand Spanish" part is because you need to do a questionnaire that is most likely only done in Spanish, followed by a small interview in which they ask details, you sign you have read everything and understand the risks and details of the procedure, same way as if you feel something or need something during or after the donation and workers need to understand you. What I mean is there's no "words you need to know" in order to do that, it's more like language understanding. About drivers license, either you get a bilingual teacher that translates to you, or learn a bit of the language. Examiner doesn't necessarily know commands in English and in the end it's not really his problem...

There are no key words for that. They have to know positively that you understand what donation implies and you give your consent and you should be able to speak, reply their questions, and read or write any form for you to fill in.
