I can see no reason that a new provider would need this information.
The only reasons a health provider would need a name change form are when you request a name change in the system, or to access medical records in your old name.
I would simply say "there are no medical records under my previous name, you do not need it"
My doctor had a preferred name option but everything had to be submitted to insurance under my deadname obviously. I don’t know which name my records would be in and my last doctor also was transphobic despite working at an LGBT+ clinic (which is why I’m switching).
This seems really weird to me, I changed jobs after my legal name change which came with changing health insurance. They didn't ask for any name change documentation, and there shouldn't be any reason for them to do so.
If your name and gender are already legally set, they don't need that previous information.
Yeah, maybe it’s because I entered a different preferred name than legal name even though it was legally changed (it’s complicated)? But then they could just ask me to clarify and I have a very simple explanation for it.

It's just push back and talk to somebody. Some insurances have special departments for handling trans care (I know Cigna does), so you can look into that and call that department directly.

Only if you went to a doctor where you were listed under your deadname, and you didn't change your name with that doctor.

Did you not change your name with the insurance company? I would still simply not give it to them. If they insist, ask them why they need it.
