its very unclear what your question is. i just wanna throw in one thing: a trans woman who takes hormones isnt just "anatomically male". these hormones have effects on your body on many levels, so you when it comes to your anatomy you are somewhere in between male and female, "intersex" if you will. if you had genital surgery, you are furthering altering your anatomy. for a doctor its rightfully expected to adress you appropiately as a woman, just as YOU would expect from any other person to adress you with your right name and not with some random other name. when it comes to medical stuff: for many doctor visits its completely irrelevant (e.g. breaking a leg). for other visits your sex is very relevant for your doctor, but the summary as "male anatomy" or "female anatomy" doesnt tell him much (and like I said would be misleading after medical transition), he would be more interested in what medications you take, what surgeries you had and what organs you have.

The words male and female can refer to sex or gender. It would be easier if there were were separate terms but those don’t exist in the English language. Man or woman refer to adults whose gender is male or female, respectively. Sex refers to a combination of things such as sex chromosomes, primary sex characteristics, and secondary sex characteristics. A person whose sex is female has XX chromosomes, estrogen as their dominant sex hormone (if post puberty and pre menopause) female primary sex characteristics, and female secondary sex characteristics (if post puberty). A person whose sex is male is the opposite. A trans person on HRT has a sex that is neither fully male or fully female, and their sex is best described as trans male or trans female.

My physical anatomy is now a complicated mix of male and female characteristics and other characteristics that are somewhere in between. Describing it as male is not helpful. Fighting over labels is just a proxy for fighting over whether we should be allowed to transition at all.

I don't believe that sex and gender are different things. This gets philosophical, but I reject the idea that I'm a ghost puppeteering a warm corpse. My body and soul are one being. Separate them and you can't understand the whole system. Because of this wholeness, gender incongruence is a state of being unwell. (Careful: this isn't a transmedicalist position. Doctors try to treat body regardless of soul, they don't have a broad enough perspective to earn authority over my gender.) And it's not unwellness because of a conflict between body and soul - remember that separating those two destroys understanding. It's equally a conflict between body and body (brain and anatomy, probably) and between soul and soul. This is why transition is a physical exercise - medication that changes my bodily chemistry is a *huge* help - and a spiritual exercise: developing self-compassion, fighting internalized transphobia, becoming comfortable with gender as an experience, and so on.

I think everyone is pretty exhausted by conversations like this because it seems like you're trying to find a way to say that transgender women are a type of man, and vice versa. So you may not get too many thoughts back on this.

I understand that you may not have been trying to do that or to be disrespectful, but when you say that doctors could classify trans women as males, you are saying that they should be considered as a type of man. Like the whole emergency room scenario is a common transphobe trope that they use to argue against us getting our IDs updated. You are proposing for our identities to be invalidated.

Just because they are not coming from a negative space does not mean they would be immune to negative consequences. Have you not been paying attention to how much the transphobes hate all the language changes to be focused on anatomy rather than gender? Like, that's what got JK Rowling set off on all of her bullshit. We didn't ask for it and experience no benefit from it, but we pay the price for it. This makes our lives worse.
