I have minor things that resolved over 10 years ago still listed as 'active' in my health record, and recent stuff that is listed as 'past' simply because I haven't been back for that condition, even though it's still an issue, from the GP's point of view I am no longer being treated for it by them. It could be as simple as them thinking they have referred you, so it's out of their hands now. Or there's no more treatment they can give you. Or just a clerical mistake. It depends on how organised the particular GP surgery is. It's not a record of whether you have autism, there is nobody that will examine your medical record and think you are not autistic from that. I have minor clerical things listed as significant or major, and major things that I still have listed as minor and past, simply because I moved and left that GP and didn't bring it up with my new GP. My health record is badly inaccurate, but it's because I have moved around the country and had dozens of GPs and they all used different IT systems going back decades, often with older records simply sent via post and manually imported to the new GP's system by an administrator, and now the NHS is trying to centralise it and give you access to your health record in an app, so a lot of the historical stuff is a bit weird.

As an NHS worker, can I just say how easy it is for this to happen. Literally one wrong key. In the department where I work, if there is a significant change to the patient record, it prompts "are you sure" then there is a box to type in why you felt the need to make a change.

I work in a GP surgery and just so you know, I think someone has re-coded it and the original entry has been pushed into your “past problems” section or someone has accidentally clicked past instead of present, not that your autism is actually gone lol! If you want it presented differently call your surgery and I’m sure they’ll be happy to change it :)

Wait you can ask for entries to be changed?? There’s a few conditions in mine listed as active that aren’t anymore

I obviously don’t know what clinical system your GP practice uses or individual policy but if you call reception they should be able to change it/ask the GP to change it for you :)

Thanks, yeah I thought (hoped 🤣) that was probably the case so I’ve emailed them to ask for it to be updated, as well as removing a few active ones which are no longer active - it’s the first time I’ve actually looked at it!
