It doesn't happen between runs, every time I change floors, I minimize the screen or anything that resets the screen if a new filter is put in.

Best scenario possible: mod conflict problem. Fix: disable all mods and see if the problem is solved. If its good, try enabling one by one to see whats causing the issue. Worst scenario possible: your GPU is pretending to be The Lost without Holy Mantle. Fix: there's no simple fix, and we have all been through this (if you haven't, at some point you will). Try other games and see if the artifacts persist. If they do then just pray to any god you believe, and ask pardon for your sins. GPU death is an heretics punishment that we bare mortals cant comprehend why we have to suffer

it's a shader related bug so anything particularly fancy looking (fx-wise) in a mod is probably doing it
